If you and your employees are already insured under a CaliforniaChoice Medical plan, just log in as a member at calchoice.com. Select Member Marketplace from the “Employee” drop-down menu. Then select your desired coverage. You can also access a Critical Care Illness brochure, which outlines all of the coverage limitations and exclusions.

The MetLife Critical Illness Insurance (CII) is a limited benefit group insurance policy. It contains certain exclusions, limitations, and terms for keeping coverage in force. Product features and availability vary by state. There may be a preexisting condition exclusion. There may be a Benefit Reduction Due to Age provision. There may be a Benefit Suspension Period between recurrences of the same Covered Condition or different covered conditions. Attained Age rates will increase when a Covered Person reaches a new age band. Rates are subject to change.
