- Dental subscribers who are enrolled and covered for the entire first month of the group’s new business effective date are eligible counts for the purpose of the bonus payout. COBRA subscribers are excluded from eligibility counts. Final counts will be confirmed approximately 60 days after the new business effective month has closed.
- For co-brokered new business, the primary agent of record listed on the employer application will be the only eligible party for bonus payout; secondary co-broker will be excluded from bonus program eligibility.
- Group must be in good standing with monthly premiums paid in full for 12 consecutive months to qualify for bonus.
- Bonus eligibility will be qualified based upon the broker and agency of record listed on the new business employer application.
- CHOICE Administrators® reserves the right to recuperate any bonus funds paid for disqualified groups based upon program eligibility.
